Résumé : Cette recherche se propose d’étudier la fabrique de soins réalisée par les professionnels de santé en contexte d’assouplissement du régime alimentaire chez les malades âgés, diabétiques, suivis au Centre Antidiabétique (CADA/INSP) d’Adjamé. Des entretiens individuels semi directifs réalisés avec sept (07) professionnels de la santé, volontaires, du CADA/INSP d’Adjamé ont été réalisés, grâce à échantillon de volontaires. Les résultats ont montré l’inventivité thérapeutique des professionnels de santé dans la prise en charge des malades diabétiques âgés. Ils ont aussi mis en relief que les tactiques de soins mobilisées par les professionnels de santé, dans la prise en charge des malades diabétiques âgés se déclinent en trois actions.  Ce qui a permis de comprendre que le diabétique âgé déstructure le model déterministe médical de prise en charge. La manipulation du protocole thérapeutique se flexibilise et devient un mécanisme de redéfinition de la relation thérapeute/diabétique âgé qui se mue en relation d’affectivité. Entre rigidité protocolaire et flexibilité relationnelle, le régime alimentaire standard se présente comme un instrument de délégitimation du protocole thérapeutique appliqué aux diabétiques âgés.

Manufacturing care for the benefit of older people at the Adjamé antidiabetic center (Abidjan-Côte d’Ivoire)

Abstract: This research aims to study the fabric of care carried out by health professionals in the context of relaxation of the diet in elderly diabetic patients followed at the Antidiabetic Center (CADA / INSP) of Adjamé. Individual interviews carried out with seven (07) health professionals, volunteers, from the CADA/INSP of Adjamé were realized. The analysis of the results revealed the therapeutic inventiveness of health professionals in the care of elderly diabetic patients. The care tactics mobilized by health professionals in the care of elderly diabetic patients are divided into three actions: taking into account the singularity of the pathological profile; taking into account the socio-cultural environment of the patient; responsibility of the patient. These different care tactics have made it possible to understand that the elderly diabetic deconstructs the medical deterministic model of care. The manipulation of the therapeutic protocol becomes more flexible and becomes a mechanism for redefining the therapist/elderly diabetic relationship, which is transformed into an affective relationship. Between protocol rigidity and relational flexibility, the standard diet presents itself as an instrument of delegitimization of the therapeutic protocol applied to elderly diabetics.
